Description
North Star Health, a newly independent FQHC comprised of multiple PCMH practices delivering team-based care in Southern VT and Southwest New Hampshire, is seeking a Registered Dietician to join our multi-disciplinary care teams and work with individuals on their personal priorities related to diet and nutrition and self-management of any chronic conditions through individual nutritional assessment and counseling services and to help raise awareness and engage our community around our services, Lifestyle Medicine program and issues of health and wellness. Some portion of work may be remote, based on schedule and patient needs.
In this role you will:
- Assess individualized nutritional, educational, behavioral, social, functional and clinical needs, facilitates goal planning, provide patient-centered education and evaluate desired outcomes.
- Provide individualized care in all aspects of nutritional counseling for self- management to patients and their families review and support patients' goals utilizing action plans for self-management and behavior change.
- Participate as a patient care team member, attending meetings and working closely with all team members to improve coordination of care
- Educate self proactively to provide a quality care and improve professional skills.
- Utilize teaching/learning strategies that are age appropriate and adapted to group and individual interventions
- Provide clinical and educational interventions with consideration of the cultural, ethical and spiritual values and needs of patients & their families. Offer Nutrition Classes in the community
- Provide recommendations regarding pharmacologic and non-pharmacologic therapy, including nutrition assessment
- Participate in self-directed work teams to evaluate existing programs, identify need for new programs, and assist in development or revision as appropriate
- Participate in QI and performance improvement efforts that enhance both clinical and operational effectiveness and efficiency
- Participate in community and professional outreach that increases overall awareness of chronic conditions
Requirements
You’d be a good fit if you have:
- RD license
- Interest in helping patients manage and prevent chronic disease, including diabetes, through diet and lifestyle
- Excellent organizational skills
- Effective and tactful written, verbal and nonverbal communication skills
- Ability to be proactive and to recognize opportunities for improvement through patient, provider or staff concerns
- professionalism, sensitivity, empathy, respect and a desire to work with a diverse population
Nice to have:
- Experience with motivational interviewing, group visits and leading classes
- Certified Diabetes Care and Education Specialist (RD/CDCES) certification or interest in earning this credential
