Mercy Hospital’s 4A Transitional Care and Discharge Unit is a 12-bed unit that will be opening permanently to reduce Emergency Department Holding and to promote better throughput. Patients in this unit consist of those who are transitioning from acute care to home or another care setting and patients who no longer require the intensity of   
acute hospital care but still need specialized medica, nursing or other care before discharge. As an LPN, you will assist fellow caregivers in providing exceptional, top-quality care to a diverse patient population. In this role, you can make a lasting difference in the lives of patients, all while enhancing your nursing skills through hands-on experience and numerous educational resources offered within the Cleveland Clinic system.

A PRN caregiver in this position works varying days from 7:00AM – 7:30PM. 

 

A caregiver who excels in this role will:  

  • Collect, document and report patient data and observations.  

  • Implement care plans as directed by an RN and contribute to care plan revisions.  

  • Administer medications and/or treatments authorized by a provider and observe patients for adverse reactions.    

  • Perform routine laboratory tests. 

  • Educate patients and family members.  

 

Minimum qualifications for the ideal future caregiver include:  

  • Graduate of an accredited Practical or Vocational Nursing Program  

  • Current Ohio license as a Licensed Practical Nurse  

  • Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross 

  • Internal candidates: one year of related nursing experience
