In this role, you will provide community-based mental health services including assessment, treatment planning, and skill-building interventions. You will work closely with clients, families, and interdisciplinary partners to deliver personalized, strengths-based support that promotes recovery, growth, and sustained success.

Key Responsibilities:
· Provide community-based mental health services to children, adolescents, and adults focused on recovery, independence, and resilience
· Develop, implement, and monitor individualized treatment plans in collaboration with clients, families, and interdisciplinary partners
· Utilize evidence-based, strengths-focused interventions including Motivational Interviewing, CBT- and DBT-informed skills, trauma-informed care, and positive reinforcement techniques
· Deliver behavioral and skill-building interventions that support emotional regulation, interpersonal functioning, and goal attainment
· Coordinate care with psychiatrists, primary care providers, schools, and community organizations to ensure continuity of services
· Prepare for and facilitate PSR group sessions aligned with individual treatment plan goals
· Maintain accurate, timely, and compliant documentation of services, progress, and outcomes
· Build and sustain strong relationships with parents, caregivers, educators, and community stakeholders
· Participate in ongoing training, professional development, and quality improvement initiatives
· Collaborate closely with case managers, therapists, and support staff to ensure coordinated, high-quality care
