Summary of Duties
As a Case Aide (Human Services Assistant 2), you will:
Work across all Aging & People with disabilities program areas as a member of the service delivery team.
Assist case managers in the delivery of long-term care and financial benefits to seniors and people with disabilities.
Conduct home visits on a regular basis for various reason which might include ensuring the development of service plans is effective, obtain and assist Oregonians with documentation needs for their long-term care services and financial benefits. Perform as an adjunct to case managers and must utilize judgement in recognizing and assessing client’s health and functional status and be knowledgeable of issues representing potential abuse and neglect.
Process and enter authorized payments for various services such as homecare worker payments, In-home Agency payments, Meals on Wheels, and Behavioral Support and Services payments, Emergency Response Systems.
Have direct contact with providers and Oregonians related to their long-term care benefits.
Coordinate and perform duties such as documentation, data entry, transportation, visitation, and contacting service providers.
This position requires accurate computer data entry of case management information.
Minimum Qualifications
One year of experience assisting professionals in one or more of the following specialized mental health/social /human services program areas:
Economically disadvantaged
Substance dependent
Mentally or physically disabled
Child abuse and neglect
Juvenile delinquent
Essential Attributes
We are looking for candidates with:
Experience maintaining confidentiality of sensitive information including financial and PHI within an office setting.
Advanced computer experience which includes creating spreadsheets with tables, graphs and formulas.
