Position Title: Nurse Practitioner – Infant High-Risk Clinic
Department: Neonatal Follow-Up / Infant High-Risk Clinic
Schedule: 24 hours /week  

Location: Cincinnati Children’s Hospital – Burnet, Crestview Hills, Union, and potentially Liberty Campus; Telehealth visits, including care for patients in surrounding states.

At Cincinnati Children’s, we are united by a shared purpose: to make children’s health better. As a Nurse Practitioner in our Infant High-Risk Clinic, you will play a critical role in the longitudinal care of medically complex infants transitioning from the NICU to home.

This position focuses on providing comprehensive follow-up care to patients from NICU discharge through approximately 2–3 years of age. You will work within a highly collaborative, multidisciplinary team dedicated to optimizing developmental outcomes, growth, and family support.

Patient Population

You will care for infants and young children with complex medical and developmental needs, including those with:

• Birth gestational age <33 weeks
• Discharge with feeding support (NG-tube or G-tube)
• Oxygen dependence at discharge
• Hypoxic-Ischemic Encephalopathy (HIE)
• Congenital Diaphragmatic Hernia (CDH)
• In utero substance exposure
• Developmental delays or concerns
• Other conditions requiring close nutritional and developmental follow-up

Key Responsibilities

• Conduct comprehensive assessments with a focus on growth, nutrition, feeding, and developmental progress
• Partner with families to support care plans, education, and long-term outcomes
• Collaborate closely with a multidisciplinary team, including:
  • Occupational Therapy
  • Physical Therapy
  • Speech Therapy
  • Registered Dietitians
  • Social Work
• Coordinate care, including follow-up on labs, diagnostics, and specialist recommendations
• Manage in-basket responsibilities (patient messages, medication refills, care coordination)
• Participate in pre-clinic planning, documentation, and administrative responsibilities

Work Structure

• 60% Patient-Facing: Direct care in clinic (in-person and telehealth)
• 40% Non-Patient-Facing: Care coordination, documentation, follow-up, and administrative responsibilities
• Clinic schedules and locations may vary by day and week

Qualifications

• Licensed as an APRN (PNP or FNP) in Ohio and Kentucky,

  Master's degree in related field. May require clinical license, certification or training OR Doctorate of Nursing Practice degree

  NICU experience strongly preferred
• Primary care or pediatric experience is also valuable
• Strong interest in nutrition, feeding, and early childhood development
• Demonstrated ability to work in a collaborative, multidisciplinary environment

JOB RESPONSIBILITIES

• Patient Care- Perform history and/or physical exams on patients presenting for care in area of expertise. Assess and diagnose illnesses, including chronic and acute conditions. Formulate, document and monitor a treatment plan. Order and monitor therapeutic interventions including but not limited to diets, laboratory, radiological and diagnostic tests as deemed necessary by patient history and/or physical exam. Provide clinical consultation, order interdisciplinary consults. Professional practice including but not limited to prescription of medications in accordance with Federal and State Law & Rules and CCHMC policy. Attains and maintains competency in subspecialty practice, skills and procedures in accordance with Joint Commission standards for focused (FPPE) and ongoing (OPPE) professional practice evaluation.

• Outcomes Identification and Planning- Develops outcomes as measurable goals that provide direction for continuity of care & continuity of care & consistency among providers. Develops, prioritizes, evaluates & modifies an individualized comprehensive plan of care. Addresses each diagnosis or issue, as appropriate. Derives outcomes & plans of care that involve the patient/family/caregiver when possible & are realistic to
