Who We’re Looking For
This Registered Nurse Care Manager plays a critical role for Residential Substance Abuse and Mental Health addressing and managing the social determinants of health that impact residents' recovery and overall well-being. This position emphasizes the integration of these determinants into care planning and management, ensuring that treatment is holistic and responsive to the complex factors influencing health outcomes.
The RN Case Manager collaborates with a multidisciplinary team to provide comprehensive care, considering the broader social, economic, and environmental factors affecting residents' health.
Responsibilities
Your responsibilities in this role may include:
- Holistic Assessment with Focus on Social Determinants of Health:
- Conduct comprehensive assessments that include evaluation of social determinants such as socioeconomic status, education, employment, housing, social support, and access to healthcare
- Identify how these factors impact residents' substance abuse and mental health conditions and their capacity for recovery
- Care Planning Integrating Social Determinants:
- Assessment: Gather data on residents' social, economic, and environmental conditions alongside their health status
- Diagnosis: Recognize how social determinants contribute to health issues and incorporate these factors into diagnostic considerations
- Planning: Develop care plans that address both health needs and social determinants, such as improving access to resources, addressing housing instability, or enhancing social support
- Implementation: Implement care plans that integrate medical, psychological, and social interventions to address the holistic needs of residents
- Evaluation: Regularly assess and adjust care plans based on changes in social determinants and residents’ progress in treatment
- Coordination with Community Resources:
- Connect residents with community resources and services that address social determinants of health, such as housing assistance, employment programs, and educational opportunities
- Collaborate with social workers, community organizations, and public health agencies to facilitate access to necessary services
- Patient-Centered Education and Advocacy:
- Educate residents about how social determinants of health affect their recovery and overall well-being
- Advocate for residents’ needs related to social and economic barriers, ensuring they receive support in areas such as financial assistance, legal aid, and social services
- Documentation and Reporting:
- Maintain comprehensive records of assessments, care plans, and interventions that include considerations for social determinants of health
- Prepare reports that reflect the impact of social determinants on residents’ health outcomes and treatment progress
- Quality Improvement and Best Practices:
- Participate in quality improvement initiatives focused on integrating social determinants into care practices
- Contribute to the development and refinement of best practices that address the social, economic, and environmental factors influencing health
- Crisis Intervention and Support:
- Address crises that may arise from social determinants, such as housing instability or financial stress, and provide appropriate interventions
