The facilitator has the responsibility of ensuring that the wraparound process is implemented with fidelity. He/she is responsible for ensuring that the phases and related activities of wraparound are completed. The facilitator is the guardian of the principles of wraparound and ensures the Theory of Change components are met. The facilitator also coordinates with HFW 
Support Partners to ensure a consistent approach to the model.

Responsibilities of a Facilitator:

  • Engaging the youth and family in planning and helping the youth and family identify and articulate their priority needs. 
  • Convening the Child and Family Team in coordination with other responsible service providers. 
  • Facilitate the Child and Family Team Meeting process to ensure the youth/family’s priority needs are met through an integrated Person-Centered-Plan (PCP)/Wrap Plan. 
  • Communicating frequently with the youth and family to identify emerging needs to add to the plan. 
  • Teaching the youth/family to manage their own services, supports, and plan. He/she begins to build youth and family self-efficacy early in the service planning process.
  • Helping the family and youth build and strengthen their natural supports.
  • Assisting the family and youth in recognizing the value of having natural supports involved in the PCP/Wrap Plan implementation.
  • Engaging the natural supports in the planning, implementing, and integrating them with services. 
  • Monitoring the PCP/Wrap Plan implementation as well as the engagement of all Child and Family Team members.
  • Demonstrating High Fidelity Wraparound Principles, Phases, Action Steps and Theory of Change
  • Other duties as assigned.

Position Requirements: Must meet the requirements of a Qualified Professional.

  • Bachelor's degree (in Human Services field) & 2+ years post graduate full-time experience with population served
  • Bachelor's degree (Not Human Services field) & 4+ years full-time experience with population served
  • OR
  • Master’s Degree or Higher & 1+ years full-time experience with population served.
  • License, Provisional License, Certificate, Registration, Permit issued by governing board regulating human service profession.

Additional Requirements:

  • Must complete HFW training curriculum approved by Trillium Health Resources and MH/IDD/SAS, and be certified as a HFW Facilitator in accordance with model expectations (or be in the process of completing training and certification).
  • Pass background check, the child and adult abuse registry checks, and motor vehicle screens.
  • Receive ongoing supervision by a master’s level mental health professional who is certified as a HFW Coach (or in process of being certified a HFW Coach).
  • Have received Motivational Interviewing training.
  • Have training & knowledge in dual diagnosis (MHSU & IDD).
  • Have received trauma informed care training.
  • Have received training in CALOCUS/LOCUS or CANS.

Definitions:

  • Human Service Degrees Include: Social Service, Sociology, Psychology, or other Human Service Degrees.
  • Populations Served Include: Mentally Ill-Child, Mentally Ill- Adult, Substance Abuse-Adult, Substance Abuse-Child

Pre-employment screening:

  • Complete criminal background
  • Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
  • Drug testing
  • Education verification and other credentialing based on position requirements.
  • Proof of employment history or references (if required)
  • Positions that require driving Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.
