🤔 About the Role:

We’re looking for a self-driven Senior Pharmacy Technician to join our exceptional Care team to manage patients with long-term conditions and medications in a Primary Care setting whilst working from home. This role is on a 3-6 months fixed term contract with the potential to extend or go permanent.

💻 What you'll be doing :

• Accurately recording and cording of patients health data on Suvera Dashboard & Practice EHR, ensuring all necessary reviews/assessments are completed
• Interpretation, Risk stratification, and Management of this data in line with standard operating procedure
• Conduct comprehensive reviews of patient information using professional and clinical judgment to identify needs, address concerns, and ensure safe and effective care, delivering lifestyle advice and referring patients to clinicians or other services as appropriate. Including but not limited to the following conditions: Non-diabetic Hypergycaemia (NDH), Hypertension, Asthma, Hypercholesterolemia
• Conduct structured medication reviews to ensure safe, effective, and appropriate use of medicines, identifying opportunities for optimisation, communicating need for change to Clinical Pharmacists
• Reviewing clinical letters and completing medicine reconciliation
• Gain informed consent for medication changes, provide patient education and counselling on prescribed medicines, including advice on adherence and managing side effects.
