Registered Nurse, OB/GYN Ambulatory Clinic Addiction Medicine Treatment Team, 32 Hours (Days)

POSITION SUMMARY:

A registered nurse in the ambulatory OBGYN clinic working in a high risk specialty clinic Project RESPECT, Boston Medical Center’s Perinatal Addiction Medicine Treatment Clinic. As an RN, you will plan an integral role in providing skilled nursing services to pregnant and postpartum patients with substance use disorders in an Office-Based Addiction Treatment (OBAT) model of care. This role involves comprehensive nursing care including completion of perinatal intakes, phone triage and telemedicine visits, clinical case management of prescriptions, and provision of barrier-free access to outpatient prenatal and recovery services. The RN position supports bridging care from the in-pateint Maternal Fetal Medicine Service and the Addiction Consult Service to the ambulatory RESPECT clinic and coordinates services for patients referred to BMC for stabilization, including in hospital titration. As a member of a multidisciplinary team, the RN collaborates with team members to provide holistic care and supports collaboration with internal providers as well as community partners.

ESSENTIAL RESPONSIBILITIES / DUTIES:

• Responsible for initial assessment and intake obtaining social, medical, mental health, substance use, and medication history, including completion of a Prenatal New (PNN) nursing education specific to RESPECT clinic protocols and treatment goals.

• Review intake assessment with physician and revising patient plan of care with the RESPECT treatment team as needed to create treatment plan reflective of a patinets unique perinatal and addiction care needs

• Review consents and treatment contracts with patients, obtaining signatures and giving patient a copy

• Provide ongoing case management of patients receiving medications for addiction treatment

• Assessment of signs and symptoms of opioid withdrawal using the COW (Clinical Opioid Withdrawal) scale

• Assessment and monitoring of patients in the induction, stabilization, and maintenance phases of treatment, including ongoing and systemic monitoring for overt and covert signs and symptoms of withdrawl or returns to use

• Ongoing education and support of patients in all phases of treatment including; nursing visits, urine toxicology screens, routine labs and as needed, medication teaching, monitoring, pill counts, observed dosing, and medication refills.

• Follow State and Federal guidelines in providing care to opioid dependent patients in collaboration with licensed, prescribing physicians and pharmacy

• Coordinate transition of care plans to primary care medical homes and providers for continuation of addiction medicine treatment

• Collaborate with social workers and peer recovery coaches to support the provision of care, including safety assessments and mandated reporting as appropriate

• Partner with the pharmacy and RESPECT Clinic team in obtaining medication orders, prescriptions, refills, and communication as needed

• Consistently using clear, concise, and effective communication written/oral and complying with departmental policies when sharing/documenting relevant patient care data. Provides follow up care in person and via telephone when needed

• Collaboration, communication, and meetings with internal addiction medicine treatment teams

• Initiate transition of care plans to primary care  

• Perform other duties as specified by the Program Director

JOB REQUIREMENTS

EDUCATION:

• Nursing degree: Diploma, ASN or BSN (preferred), Ability to obtain BSN within 4 years

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

• Registration and current RN licensure in the state of MA in good standing required.

• Requires current basic life support (BLS) certification. Some units may require Advanced Cardiac Life support (ACLS) certification.

• Additional certifications may be required for certain practice areas

KNOWLEDGE AND SKILLS:

• Basic computer proficiency inclusive of ability to access, enters, and interpret computerized data/information.

• Effective interpersonal skills to facilitate communication with various members of the health care team, patients, and families.

• Organizational skills to set priorities and efficiently complete assigned work.
