Operating 24 hours, 7 days a week, we offer reliable, dignified care in your own home and specialize in health services, personal care, and champion services.

As the Social Worker, you will be responsible for providing direct social services in the home, and working with the patient's family or support system to provide, plan and coordinate social work services. The Social Worker also works with other professional staff to assess psychological needs and to establish goal directed contracts with patients, families and significant others to resolve medical social work needs of patients.

Responsibilities Include:

• Develop plan of care for patient including assessment findings, frequency of visits, expected outcomes and short-and-long term goals
• Assess the psychosocial needs of patients and develop a therapeutic plan of care to address social work needs
• Implement plan of care in collaboration with interdisciplinary care team
• Observes, records and reports to the physician and program personnel, the patients' reaction to treatment and any changes
• Documents all treatment sessions

Position Requirements:

• Master's degree in social work from a school accredited by the Council of Social Work Education and the Department of Education.
• Licensed Social Worker in NYS (LMSW or LCSW)
• Prior social work experience experience in a health care setting as a member of a multi-disciplinary team.
• Home Care experience preferred
• Access to reliable transportation and able to visit patients in their homes in the following boros: Brooklyn or Queens
