Clinical pharmacy is a key driver for reducing pharmacy costs and improving patient health outcomes. This position provides direct patient care blended with the opportunity to work with SLPG leadership to develop the role of population health pharmacy within SLPG. This position will have a blend of on-site clinical and administrative work and the opportunity for telework.Uses evidence-based outcomes to improve medication therapy and chronic disease management for individuals covered by value-based programs in which SLPG participates. Establishes, implements, and evaluates medication management policies, procedures, clinical care, and data collection for patients under the care of SLPG practices. The clinical pharmacist will collaborate with physicians, other clinical pharmacists, and other clinical team members to deliver care while shaping out processes to improve the quality of care across a clinically integrated ambulatory environment.

WORK PERFORMED
JOB DUTIES AND RESPONSIBILITIES:
- Conducts patient medication history review and formulates an individualized patient medication plan which includes lifestyle modifications and address barriers to adherence.
- Provides an organized and comprehensive approach to consultations for medication management including indications, medication selection, medication dosage, access to medications, drug reactions, and drug interactions.
- Leads initiatives to optimize medication use across the network such as reducing the use of high-cost, low value medication prescribing, coordinating medication conversions with prescribers and optimizing the use of the electronic health record.
- Responsible for physician consultation and recommendations. Working with SLPG leadership in consultation with SLUHN pharmacy leadership, develops and maintains policies and procedures related to mediation management based upon his/her clinical experience in our ambulatory settings.
- Serves as a resource and liaison for implementation of drug management initiatives across SLPG. Working with physician leadership, develops protocols and pilots them to initiate, adjust, monitor, and recommend drug therapy.
- Functions as a direct-patient-care clinical pharmacist in these initiatives. Collects, analyzes, interprets, and forecasts prescribing habits based on current patterns from medication utilization reports.
- Collaborates with clinical and administrative staff to determine need for process or system improvements related to medication utilization, including polypharmacy, opioid utilization, value-based prescribing, and optimization of medication therapy for select chronic conditions including heart failure, diabetes, hypertension, and COPD/asthma.
- Maintains an active leadership role on select committees and workgroups identified by SLPG leadership.

- Working with physician leadership, develops protocols and pilots them to initiate, adjust, monitor, and recommend drug therapy. This includes use of the patient medical record, review of laboratory results and direct patient contact. Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.
