About the role:
As the Sr. Associate on Accompany Health’s Clinical Innovation and Population Health Team you will be:-An early part of a growing company defining the future of in-home value based care for populations with complex healthcare needs.-A key member of our central Clinical Innovation and Population Health team designing and implementing new interdisciplinary services that support high quality, proactive care for our patients. -A swiss-army knife who works across projects to solve complex problems and support decision making on the fly.-A collaborator on a team that partners with our market leadership, product and engineering teams, and frontline care team members to continuously improve our care model to meet the needs of our patients and staff.

Responsibilities will include:

  • Support our clinical and operational leaders in designing and implementing new services, from our RN chronic disease program, Pharmacy medication adherence program, and Community Health Worker support, to new virtual primary care offerings.
  • Organize clinical innovation sprints from design to implementation, incorporating user experience design and quality improvement principles.
  • Map workflows, create training materials, and develop evaluation plans for new care delivery initiatives
  • Leverage data to identify opportunities, propose options to address, and implement solutions directly with frontline teammates.
  • Support data-driven strategic planning and roadmapping for clinical innovation and population health teams.
  • Collaborate with our Product team when solutions and projects require new feature development.
  • Other projects as assigned.

Desired skills and experience:

  • Required
  • Bachelor’s degree
  • 2+ years of experience in strategy and operations, consulting, or other related functions with a focus on healthcare
  • Ability to build the trust and confidence of a wide range of stakeholders
  • Organized and detail-oriented, with a strong ability to add structure within ambiguity
  • Able to simultaneously manage multiple priorities
  • Clear and concise communicator

  • Preferred 
  • Experience in value based care
  • Experience in design, quality improvement, or product
  • Experience building program operations, including workflows and dashboards
  • Experience in data analytics and modeling
