Your Role

The Clinical Care Navigator is a member of aptihealth’s Integrated Behavioral Health Team embedded within the acute care settings of our Health System Partners at Albany Medical Center.  The Clinical Care Navigator is responsible for building relationships with referral contacts across the hospital (Discharge Planners, Floor Nurses, Case Workers, Social Workers, etc.) to facilitate referrals to aptihealth. The Clinical Care Navigator will also be proactively engaging with patients referred to aptihealth from the Emergency Department or other acute care settings.

The Clinical Care Navigator will ensure a best-in-class start to the patient’s behavioral healthcare journey by educating the patient on aptihealth services, facilitating registration, screening, and scheduling the patient into care prior to discharge from the hospital. Additionally, the Clinical Care Navigator will facilitate the diagnostic interview with clinically appropriate patients to ensure a seamless transition into aptihealth’s care.  Our goal as a team is to reduce the barriers and challenges that patients face in their search for care and to facilitate frictionless referral and transitions of care processes.

Requirements

  • Licensed Clinical Social Worker (LCSW)
  • Must possess an unrestricted New York license
  • 3+ years’ experience working in a clinical patient facing healthcare setting, preferably in acute care, inpatient behavioral health, community-based organizations, or substance use treatment facilities
  • Knowledge and competency regarding diagnosis, assessment and treatment of behavioral health diagnoses
  • Experience and comfort using technology such as a computer, virtual visit technology, and EMR platforms to document patient interactions and assessments, register, and schedule patients for appointments
  • Strong interpersonal communication skills, written communication skills, attention to detail, and active listening abilities
  • Outstanding organizational and time management skills
  • Experience working in a team environment and ability to effectively collaborate with other team members on tasks, duties, and/or projects
  • Passionate about being a trusted partner to transform behavioral healthcare
  • Understanding of healthcare processes such as discharge planning
  • Results driven -- documented success in exceeding goals and objectives
  • Ability to thrive in a fast paced, constantly changing environment

Responsibilities

  • Work directly with patients and their acute-setting care team to connect them with the behavioral healthcare providers and resources they need
  • Provide appropriate information, education, and explanation of aptihealth services to patients displaying a comprehensive understanding of aptihealth’s mission, values, and care model and the ability to communicate it in a clear, compassionate way
  • Understand how insurance works and be able to effectively verify insurance eligibility and communicate patient cost sharing responsibility
  • Facilitate the beginning of the patient’s behavioral healthcare journey using aptihealth’s platform
