The 61st Street Service Corporation, provides administrative and clinical support staff for ColumbiaDoctors. This position will support ColumbiaDoctors, one of the largest multi-specialty practices in the Northeast. ColumbiaDoctors’ practices comprise an experienced group of more than 2,800 physicians, surgeons, dentists, and nurses, offering more than 240 specialties and subspecialties.
Job Summary:
The incumbent will provide case management services to the patients in the nephrology clinic. The incumbent will be the main point of contact for established patients with chronic kidney disease. The Patient Navigator builds strong relationships with kidney disease patients to help them stay engaged in medical care and adhere to their medications. Patient Navigator is committed to removing the patient's barriers to care by identifying critical resources, helping them navigate through health care services and systems, and promoting health. They work closely with the Care Team, including doctors, nurses, and other clinical staff to support positive health outcomes.
Job Responsibilities:
- Greeting and interacting with patients.
- Ability to work flexible hours/days
- Check patient in via EPIC; accurately collecting, updating, and verifying patient demographic and insurance information in EPIC.
- Collecting referrals (as needed) and ensuring accuracy of information on referral forms.
- Performing insurance eligibility and verification as needed.
- Collection of co-payments/fees and posting of payments to EPIC, daily reconciliation of the cash drawer.
- Collection and recording Pharmacy information and Meaningful Use data in EPIC.
- Obtaining insurance pre-authorizations for treatments, diagnostic procedures and medication as needed.
- Schedule new/ follow up appointments according to provider’s participation status and help coordinate follow up care and diagnostic testing, providing patient with all necessary instructions to ensure a smooth follow up visit.
- Accurately scanning information into EPIC (including but not limited to insurance card, referrals, registration forms, outside records, etc.).
- Helping with gap closures: monitor care management programs’ phone line, triaging patients to appropriate care managers and providers to fill gaps in care.
- Increases continuity of care by managing relationships with referring provider, transitions care from diagnostic services to procedural services as required.
- Attends and represents the organization at professional conferences, in-service training, and meetings at the request of or with the approval of supervisor
- Inquire about medication usage, refills, and REMS enrollment.
- Assist with patient’s paperwork for transition to dialysis.
- Communicate with Care Team members to facilitate patient care
- Provide cross coverage as needed
- Checking messages and following up with patients (including reminder phone calls).
- Responsible for managing a smooth clinic flow by assessing and anticipating MD schedule, ensuring any patient results or chart documents are available and assigning clinic space effectively.
- Ensures that the examination /consultation/treatment rooms are stocked with requisitions, forms, and medical supplies
- Assist patient with transport when necessary
- Responsible for making sure the exam rooms and waiting area are cleaned and maintained.
- Interviews patient, measures vital signs, height and weight, and records information in EPIC
- Medication reconciliation and refill requests to be entered into EPIC for verification by MDs
- Completes medication reconciliation and/or test requisitions and places order in EPIC at direction of provider
- EPIC Order Entries Required.
- Performs EKGs, phlebotomy, and urinalysis
