Role Description
At Oak Street, Population Health drives both our clinical and economic model and is focused on delivering high-quality and cost-effective care.
The Sr. Associate, Population Health reports to the Director of Population Health.
The ideal candidate is a self-starter, enjoys solving challenging problems, uses data to drive insights, and excels in a fast-paced environment. This will be a high-visibility role requiring significant cross-team collaboration within Population Health and other corporate teams (e.g., Business Intelligence, Care Services, Clinical Quality, Managed Care Operations, etc.).
Responsibilities
Supports a high-impact care model initiative focused on patient engagement.
Performs data management in spreadsheets and analyzes data to identify high level program insights
Builds program materials (slide decks, one pagers etc) to communicate impact to internal team and key stakeholders.
Communicates effectively with internal team members along with clinical and operational field leaders.
Other duties, as assigned.
What are we looking for?
Strong interpersonal skills that support effective stakeholder engagement
Strong written communication skills; proficient with Microsoft PowerPoint
Analytical and data savvy; proficient with Microsoft Excel
Passion for excellence, willing to go the extra mile to achieve objectives and ensure consistently high quality work product
Quick learner comfortable with a high level of ambiguity
Ability to succinctly summarize and synthesize large amounts of information
Strong accountability and discipline against timelines and deliverables
Collaborative working style, able to work cross functionally
Bachelor’s degree required
US work authorization
