We are currently in need of a Lead Licensed Vocational Nurse (LVN) that is dedicated to improving the overall health of our community by providing medical care for the entire family.
HSNT is seeking a Lead LVN that is passionate about providing care, education, and resources in a community health setting to patients and families that have very limited options for their medical needs; and has a desire to serve as an integral leader—the clinical “go to” person--in HSNT’s medical center.
Our Lead LVN supports patient care in a fast-paced environment, with a team-based, collaborative approach, within a non-profit, community healthcare setting. HSNT treats patients from all walks of life and have a direct impact in our patients’ lives regardless of socio-economic status--for a wide range of acute and chronic physical, mental, and social health conditions and in collaboration with a team of medical professionals.
A day in the life of our Lead LVN may look like this:

  

  • Supervises clinical support staff, Medical Assistants Ensures training needs are continually assessed and adjusted based upon the needs of the clients and providers. 
  • Performs periodic audits of clinical documentation, process, and procedures. 
  • Provide orientation and training to nursing and medical assistant staff at clinical location. 
  • Support clinical providers by completing vital signs, initial history/chief complaints, procedure set-up, etc. for the providers assigned patients. 
  • Perform nurse visits for medication administration, refills, and patient education. 
  • Perform phlebotomy using appropriate techniques and equipment, process specimens for shipment to the lab to ensure specimen integrity for diagnostic lab testing. 
  • Call or fax prescriptions to designated pharmacies by clinical provider’s orders. 
  • Schedule patient appointments, referrals and follow ups. 
  • Educate clients on medication regiments and drug adherence. 
  • Manage and maintain pharmacy, including ordering and inventory. Oversee vaccine inventory and reporting. 
  • Manage the OSHA, MSDS programs for HSNT medical center. Serve as Lead Emergency Response Team Lead, conduct Fire and Safety Drills. Maintain client records in compliance with HIPAA and agency standards. 
  • Facilitate referrals to other medical professionals as directed by clinical providers. 
  • Perform telephone triage as needed. 
  • Participate in Quality Management studies including reporting and implementation of clinical interventions to address the needs of the patients. 
  • Perform additional duties as assigned by the Senior Clinical Nurse Manager. 
  • Regular in-person attendance is required as an essential function for this position. 
  • Must have the ability to work efficiently under stressful conditions and or environment in a timely manner. 
  • Ability to operate and drive a vehicle safely during work hours.



Requirements

 We ask our Lead LVNs to possess a minimum of:

  • Strong clinical skill set with ability to serve as a knowledge expert to other medical support staff, including serving as clinical electronic health record subject-matter expert, super user. 
  • Must have proven skills in working independently, self-motivated, goal oriented, strong initiative, and be a self-starter. 
  • Ability to organize and prioritize. 
  • Strong communication skills both verbal and written. 
  • Bilingual skills (Spanish/English) a plus. 
  • Proficiency with data entry (including familiarity with electronic health record systems). 
  • Strongly developed attention to detail. 
  • Ability to adapt easily to changing environments. 
  • Ability to multi-task in a busy environment. 
  • Ability to communicate effectively with all levels of the organization. 
  • Ability to work with people and work under pressure. 
  • Ability to maintain confidentiality. 
  • Ability to relate to individuals from a variety of racial, ethnic, religious, and cultural/socioeconomic backgrounds, including various sexual orientations. 
  • Ability to work at any HSNT location, other than assigned site and ability to work weekends, as responsibilities may dictate. 


  Education and Experience: 


  • Education: Advanced vocational education required to obtain licensure as a LVN or higher 
  • Must have current license as a Vocation Nurse in State of Texas 
  • Previous supervisory experience in an outpatient
