Job Overview

We are seeking a compassionate and detail-oriented Registered Nurse (RN) to conduct initial patient assessments for our home care agency. The RN will play a key role in evaluating patient needs, developing care plans, and ensuring compliance with state and federal regulations.

Responsibilities

  • Perform comprehensive initial assessments of patients in their homes.
  • Develop individualized care plans in accordance with physician orders and regulatory guidelines.
  • Document assessment findings accurately and timely in the electronic health record (EHR).
  • Educate patients and families on care needs, safety precautions, and available services.
  • Coordinate with physicians, caregivers, and agency staff to ensure continuity of care.
  • Ensure compliance with state, federal, and agency standards of practice.

 Qualifications 

  • Current and active Registered Nurse (RN) license in the state of Georgia.
  • Minimum 1 year of clinical nursing experience (home health experience preferred).
  • Strong assessment, critical thinking, and communication skills.
  • Ability to work independently and manage time effectively.
  • Reliable transportation and willingness to travel within the 30125 service area.
