Description
The Senior Care Manager conducts and schedules assessments, referrals, advocacy and supports, counseling, education of patients and enrollees and care team members assuring the patient receives quality services to maintain optimum healthcare and life needs without barriers.
Shift: Monday-Friday 8am-4:30pm
Essential Job Functions:
- Assists and supports program supervisor with collecting and maintaining data and tracking: Specifically, client enrollment, discharge, HARP, HealthHomePlus and mandatory reporting associated with each.
- Assures effective compliance with processes and policies by making sure Care Managers are effectively maintaining client records.
- Assists program supervisor with providing instruction and guidance to care managers on effective client care.
- According to CNYHHN polices conducts outreach, Enrollment and Assessments and collaborates with the program manager to ensure each client transitions to their care manager with all necessary documentation and assessments completed correctly and on time.
- Provides supervisory support in the absence of the Program Supervisor in directing and supporting the team to achieve effective and efficient daily tasks.
- Outreach and engagement to formally enroll referred individuals into the care management program.
- Conducts assessments, evaluates needs, establishes and maintains care plan and maintains referrals for enrollees.
- Assures supports are in place inclusive of peer and family contacts.
- Develops Interim Plan of Care based on preliminary clinical information and assigned level that will identify linkages and services immediately required, based on information received from referral sources if applicable.
- Ensures all initial linkages are established and maintained.
- Collaborates with all services providers and establishes team communication plan.
- Monitors goals on a continuing basis and that team is communicating.
- Monitors that the care plan is relevant to health home policies and procedures.
- Consults with family members and social supports to maintain support consistency.
- Advocates for additional services and linkages as appropriate.
- Maintains current care management documentation and information regarding care management activities within the required health IT system.
- Will be required to perform other duties as requested, directed or assigned.

Requirements
Minimum Job Qualifications:
Bachelor’s degree in psychology, sociology, human services or related field or equivalent experience.
And
1-2 years’ experience in Human Services, primarily Mental Health, Substance Abuse and medical information.
And
Must possess the ability to make independent decisions when circumstances are warranted
And
Basic Computer Skills (Windows, Outlook, Adobe Acrobat, Word, Excel).
And
Support a person-centered approach to care and treatment.
And
Must subscribe to a Professional Canon of Ethics.
Other Job Requirements: Must maintain a valid Drivers License, as specified in the personnel policies. Must maintain all laws and policies concerning confidentiality, security, and client rights as specified in the employee handbook, 42 CFR Part 2 and HIPAA. Must maintain current certifications and/or trainings as specified by State law and agency regulations.
