Clinical Outpatient Psychiatric Nurse Practitioner - Per Diem

Conifer Park is currently seeking a Per Diem Psychiatric Nurse Practitioner for our Outpatient Clinic in Troy, NY;

As Per Diem Nurse Practitioner with Conifer Park Outpatient, you will be responsible for providing psychiatric evaluation and care to patients for existing or newly diagnosed psychiatric illness that interferes with patient’s substance use disorder treatment.

Additional Responsibilities:

Performs comprehensive psychiatric examination, determines initial diagnosis, and records in patient chart. Develops collaborative practice agreement with physician, if required. Orders diagnostic tests, such as laboratory tests needed to manage patient care related to psychiatric diagnosis or prescribed medications; reviews and interprets test results. Develops, implements and documents patient management plans, medication and therapy recommendations, and assists planning of continuity of care. Performs and documents follow up visits with updates in diagnosis and medications, as needed. Assesses and manages effects and side effects of medications. Instructs and counsels patients regarding compliance with prescribed therapeutic regimens and treatment of substance use disorder. Performs pre-admission clearance for patients with psychiatric symptoms and conducts pre-admission interviews when indicated. Participates in triage activities for the psychiatry department. Evaluates and treats psychiatric emergencies, including response to codes. Conducts emergency evaluation of suicidal patients and provides direction of appropriate status and precautions. Refers unstable patients to appropriate psychiatric facilities. Participates in individual case assignments, case conferences, multidisciplinary treatment team meetings, educational presentations for staff and patients, medical staff meetings, patient care monitoring conferences, and additional duties as assigned by the Medical Director.

Requirements

Post Graduate Degree, Masters in Psychiatric Nursing, Current NYS Nursing License, Certified Nurse Practitioner - Psych; Minimum of 1 to 3 years of experience as a Psychiatric Nurse Practitioner (Preferred); Experience in the treatment of individuals with substance use disorders (Preferred); Active Medicaid Number (Highly Preferred)
