The Texas Child Health Access Through Telemedicine (TCHATT) Senior Pediatric Mental Health Care Coordinator will work as part of an interdisciplinary, team-based program that cares for patients enrolled in the TCHATT program at Dell Medical School. TCHATT provides telemedicine or telehealth programs to school districts to help identify and assess the behavioral health needs of children and adolescents and provide access to mental health services. This position will work to coordinate TCHATT projects and administer the TCHATT program, ensuring alignment with legislative, institutional, and program goals.
This is a state-funded position sponsored by the Texas Child Mental Health Care Consortium (TCMHCC). TCMHCC was created by the 86th Texas Legislature to leverage the expertise and capacity of the health-related institutions of higher education to address urgent mental health challenges and improve the mental health care system in this state in relation to children, adolescents, and perinatal women.
This position has an end date of 8/31/2025, but may be renewable based upon availability of funding, work performance, and progress toward goals.
Purpose
The Senior Pediatric Mental Health Care Coordinator will lead mental health careĀ coordination for the TCHATT program. The purpose of this position is to help children and families navigate through the TCHATT program and assist in connecting to providers in the community for long-term care, if needed. Interest in developing sustainable pathways for referrals and enhancing processes is welcomed.
Responsibilities
Connect students and families to community resources for long term care, providing ongoing support and facilitation to ensure effective transition. Coordinate with TCHATT professionals, schools, and families to minimize obstacles and increase patient and provider satisfaction. Maintain confidentiality of information as appropriate in all aspects of care.
Lead, manage, and educate on direct service pathways for internal referrals as part of ongoing program growth. Provide support to clinical staff on utilization of internal resources.
Serve as the lead care coordinator for internal clinical and consultation tracks. Collaborate with supervisor and colleagues to develop and implement needed program development, project implementation and process improvements both internal and external. Sample projects could include creating and updating internal referral processes, identifying service gaps for specialized populations and creating TCHATT pathways, training of Texas Child Health Care through Telemedicine (TCHATT) campus liaisons or supporting various outreach and recruitment initiatives.
Ensure clinical documentation is available for reporting by (1) monitoring compliance within secure, data collection software systems, such as REDCAP, Trayt or other; (2) updating and supporting EMR integration such notes and orders; and (3) implementing other documentation and technology-related special projects. As assigned, may serve as a liaison for data and data-related projects.
Lead and manage community partnerships and referrals with identified agencies to ensure ongoing functioning of pathways.
Hire, train, and supervise (1) Graduate Assistant for the direct service pathway managed by the Senior Care Coordinator. Lead a project managed by a Graduate Assistant.
Work in an inter-professional model and routinely collaborate with pediatric psychiatrists, psychologists, other licensed professional counselors and administrative staff internally to understand the long-term psychological needs of individual patients. Assembling information concerning a patient's clinical background and referral needs.
Work with supervisor to create and implement onboarding training for new Care Coordinators and other employees.
