SUPERVISOR: SSN/CHR Coordinator
FLSA: Non Exempt
SALARY RANGE: $13.00- $18.00
ALL FULL TIME POSITIONS: In addition to your pay, your salary also includes Medical, Dental and Vision Insurance. The Biweekly deduction from employee paycheck is $40.00 for Employee Only, $95.00 Employee plus Children, $120.00 Employee plus Spouse, and $140.00 Employee plus family.
JOB SUMMARY: Serves as an advocate for patient needs, performs in home supportive services, provides information about available health programs. Participates in community health promotion and disease prevention events.
CPN Health Services personnel may be assigned to provide services to patients of all ages including but not limited to Indian Children and will require a criminal history background screening.
ESSENTIAL FUNCTIONS:
- Performs assigned supportive procedures for the well-being of patients
and accurately records in patients chart.
- Observe patients signs and symptoms for reaction to drugs and treatments and reports to Coordinator.
- Accurately record nursing needs of patients or nursing care plan as directed by the Coordinator.
- Patient advocacy
- Personal care for patients, food preparation, shopping, assistance with bathing and feeding patients, running errands, and minor house cleaning.
ADDITIONAL DUTIES:
- Perform other duties and projects as required by management.
- Make home visits ordered by physician or SSN Project Director.
- Participates in the community health promotion and disease prevention events.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Ability to communicate effectively and accurately with patients and staff.
- Ability to maintain strict confidentiality regarding all patients encounters.
- Computer skills
- Specialized knowledge, skills, and abilities to successfully perform duties of
the position.
- Knowledgeable of basic Medical Terminology
MINIMUM QUALIFICATIONS FOR CONSIDERATION:
- Certified or willing to be certified as CHR upon request within one year of employment.
- Certified in Basic Life Support (CPR) or becomes certified within 3 months of employment.
- Knows how to take patient vital signs and is familiar with normal medical values for vital signs.
- Trained to complete Blood Glucose Testing and familiar with normal medical values and when to report to the Coordinator.
- Experience in a clinic setting preferred.
- Must have a valid driver’s license and driving record that allows access to CPN vehicles for daily work function.
- Computer skills required for documentation in the patient record.
PHYSICAL REQUIREMENTS:
- Sitting and standing for prolonged periods of time.
- Some lifting, bending, stretching, and kneeling may be required.
ENVIRONMENTAL CONDITIONS:
- General clinical and home environment with frequent exposure to chemicals, blood borne pathogens and patient body fluids.
