Social Worker, BSW – Long‑Term Care (LTC)
Location: Fond du Lac, WI
Schedule: Monday–Friday | 8:30 AM – 5:00 PM

About the Role
We are seeking a compassionate and collaborative Social Worker (BSW) to support residents in a long‑term care setting. This role focuses on resident advocacy, care coordination, and collaboration with interdisciplinary teams and community partners to ensure residents and families receive the support and resources they need.
Key Responsibilities
- Conduct resident interviews to assess needs, priorities, and goals
- Document case activity and support individualized treatment plans
- Collaborate with interdisciplinary care teams and community resources
- Assist residents and families with referrals and access to community assistance
- Build and maintain relationships with community health, welfare, and social service agencies
- Identify opportunities to improve standard work practices
- Maintain safety awareness and perform other duties as assigned
Required Skills & Qualifications
Technical Skills
- Proficiency with Point Click Care (PCC), EPIC, and MDS
- Working knowledge of Minimum Data Set (MDS)
- Understanding of State and Federal compliance requirements
Interpersonal Skills

- Conflict resolution and de‑escalation
- Patience, persistence, advocacy, and persuasion
Education & Experience
- Bachelor’s degree in Social Work (BSW) required
- No prior experience required
Physical Requirements
- Frequent sitting, standing, walking, reaching, and keyboard use
- Ability to use vision and hearing for effective communication and safety awareness
- Occasional bending, stooping, twisting, lifting, and driving
Licensure / Certification
- No professional license or certification required

PRIMARY RESPONSIBILITIES
- Interviews residents, assesses priorities and documents case activity.
- May consult with members of the multi-disciplinary team in liaison with community resources to support the resident's treatment.
- Assists residents/families with obtaining community assistance by referral to proper resources.
- Develops and maintains working relationships with community health, welfare, and social agencies and seeks creative means to assist the resident with his/her needs.
- Active participant with other members of the team regarding opportunities for improvement in standard work.
- Works in a constant state of alertness and safe manner.
- Performs other duties as assigned.

- Bachelor's degree in Social Work

PHYSICAL REQUIREMENTS
- Frequent reaching, sitting, standing and walking.
- Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more).
- Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
- Frequent keyboard use/data entry.
- Occasional bending, stooping, climbing, twisting and gripping.
- Occasional lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
- Occasional use of vision to identify and distinguish colors.
- Occasional driving.
- Rare repetitive foot/leg and hand/arm movements and squatting.
