Job Summary

Summary
Responsible for providing nursing care to patients in their homes.<Does this position require Patient Care?Yes
Essential Functions
-Assessing patients' health status and developing individualized care plans.
-Administering medications and treatments as prescribed by the supervising registered nurse or physician.
-Monitoring patients' vital signs and reporting any changes to the supervising nurse or physician.
-Assisting patients with activities of daily living such as bathing, grooming, and dressing.
-Providing wound care and dressing changes.
-Performing basic diagnostic tests such as blood pressure checks and glucose monitoring.
-Educating patients and their families on their medical conditions and self-care techniques.


 

Qualifications

Education
Bachelor's Degree Related Field of Study required

Can this role accept experience in lieu of a degree?
No

Licenses and Credentials
Class D Passenger Vehicle Driver's License MASS -required

Licensed Practical Nurse [LPN - MASS -Required

Experience
Completion of a state-approved practical nursing program and a current LPN license. required

Knowledge, Skills and Abilities
- Basic knowledge of medical terminology and procedures.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate and patient demeanor.
- Reliable transportation to travel to patients' homes.
