VNA Manager Clinical Referrals & Liaisons

This position is a member of the Visiting Nurse Association’s (VNA) management team and is responsible for oversight of hospice liaisons, community liaisons and the agency’s intake and registration department. The manager is responsible for assuring that the teams provide service in an effective and efficient manner, and that responses to all inquiries for the VNA’s services are processed in a timely manner. Manager promotes and maintains positive relationships between referral sources and the VNA’s liaisons and intake teams.
The role also oversees the coordination of activities and events that serve to promote the VNA’s services throughout community

ESSENTIAL FUNCTIONS:

  • Manages the hospice liaisons. Ensures effective and efficient response to all hospice inquiries.
  • Manages the agency’s community liaisons.  
  • Oversees daily operations of the Intake and Registration team to assure that referrals are processed in a timely manner.  
  • Oversee all hiring, training, and dismissal if appropriate of clinical resource and intake team and all liaisons to meet all the referral needs of the agency.   
  • Monitors daily, weekly and monthly referral metrics that are meaningful and contribute to continuous performance improvement.
  • Maintain relationships with referral sources to increase loyalty to the VNA of St. Luke’s and its brand identity.  
  • Responsible to adhere to all federal, state and accreditation rules as they relate to referrals for the VNA’s programs. 
  • In cooperation with all VNA departments responsible for service recovery activities for internal and external customers.
  • Maintains confidentiality of all materials handled within the Network/Entity as well as the proper release of information.
  • Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements. Demonstrates/models the Network's Service Excellence Standards of Performance in interactions with all customers (internal and external).
  • Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.
  • Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.
  • Complies with Network and departmental policies regarding attendance and dress code.

OTHER FUNCTIONS:

  • Knowledge of agency's practice, policies and procedures.
  • Ability to work as part of the VNA Management Team
  • Ability to communicate effectively, both orally and in writing to patients, families, physicians, referral sources and related parties.
  • Other related duties as assigned.

ADDITIONAL REQUIREMENTS:

  • Possession of a current license to operate a motor vehicle.
  • Must have access to personal vehicle for agency business purposes.
  • Must have a wireless phone

PHYSICAL AND SENSORY REQUIREMENTS:

Medium Work

Exertional activity

  • Occasionally lift and/or carry 50 lbs
  • Frequently lift and/or carry 25 lbs
  • Being able to frequently lift or carry objects weighing up to 25 pounds is often more critical that being able to lift up to 50 lbs at a time
  • Stand and/or walk at least 6 hours in an 8-hour workday
  • Very few medium occupations in the national economy are performed primarily in a sitting position

Nonexertional activity

  • Stooping (bending at the waist) and crouching (stooping and bending at the knee) – required frequently in most medium jobs
  • Handling (grasping) – required frequently in most medium jobs
  • Fingering (fine manipulation) – required only occasionally in most medium jobs

Important:  The functional capacity to perform medium work includes the functional capacity to perform sedentary and light work
