About the role

Overview

Provides assessments, health education, counseling, advocacy and limited case management to Guilford County at risk populations. Congregational Nurse will function within the locations that impact the underserved focusing on health promotion, disease prevention, and chronic disease management. Congregational Nurse will partner with Congregational Social Work Education Initiative students assigned at the various sites in coordination of services.

Responsibilities

Demonstrates excellent interpersonal skills in working with at risk populations, healthcare providers, and community agencies.Plans health programs in conjunction with CSWEI (Congregational Social Work Education Initiative) students to address wellness focus and disease prevention and chronic disease management. Provides oversight for CSWEI students and Community Health workers as applicable.Facilitates and/or conducts individual and group health education activities. Organizes, coordinates, promotes health screenings and/or support groups.Provides personal assessment and counseling to individuals/families on wholistic health matters.Advocates on behalf of clients to secure a primary health care home and connect with community resources. May serve in a case management model.Works with individuals to establish eligibility for health insurance, Medicaid, Medicare, Disability or Orange Card.Works with individuals to secure appropriate prescribed medications and maintain compliance. Assess Social Determinants of Health and make appropriate referrals.Participates in educational and network programs as arranged by the Cone Health System.Works to meet the focus area guidelines set by grant funders such as the Cone Health Foundation, Cone Health, and the Congregational Nurse Program. Maintain records regarding all activities ? documenting in Epic system and submitting written reports on a monthly and annual basis to the Cone Health Congregational Nurse Program.

Qualifications

EDUCATION:Required: Graduation from a BSN, ADN or Nursing Diploma program - consideration may be given to RN?s holding acertificate from approved foreign nursing programs. All registered nurses without a BSN or higher will berequired to enroll in a BSN program within one year of their hire date with completion of the programoccurring within four years of hire. EXPERIENCE:Required: Five or more years of clinical experience in a health care setting.Preferred: Experience in teaching and/or health counseling is highly desirable. LICENSURE/CERTIFICATION/REGISTRY/LISTING:Required: Current NC Board of Nursing RN license or compact state license; BLS (CPR)-American Red Cross or AHA Healthcare Provider
