Intake LPN - Providence Infusion and Pharmacy Services
Schedule- Full Time/ Days
This position serves as member of the Infusion Intake Team, responding to incoming referrals and informational calls regarding outpatient / home infusion services. The Infusion Intake LPN is responsible for collecting clinical information in order to ascertain the appropriate services for the individual referred. This position is responsible for assisting the patient, referral source and any other callers with infusion services, and facilitates coordination of services with the patient, from the initial referral through the duration of therapy and discharge from infusion services.

Required Qualifications:
- Education sufficient for licensure
- Upon hire: Washington Licensed Practical Nurse
- Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information
- 2 years Direct clinical experience, preferably with 1 year of home infusion field experience
Preferred Qualifications:
- 1 year Office experience in health care setting
- Computer experience with infusion software, electronic medical records, Microsoft Word, and Excel
