Description:

Facilitate discharge planning to the next most appropriate site of care.

The Social Worker or Care Transition Manager is responsible for all aspects of the discharge planning process. Responsible for facilitating discharged patients to the next most appropriate site of care. Responsibilities also include connecting patients with resources, completing face to face assessments and acting as a liaison between patient's, families and outside facilities.
