The Senior Clinical Quality Assurance Coordinator: is responsible for final Quality Assurance review of LiveOnNY Organ and Tissue donor records to maintain regulatory compliance for accreditation, certification, and federal/state/local and other donation/transplantation requirements; aligns individual goals with departmental and organizational strategic goals/Key Performance Indicators for the purpose of improving organ/tissue donation rates throughout LiveOnNY’s Donor Service Area. 

 

 

This is an in-office role. Must be able to work rotating holiday and/or weekends as needed. 

 

Responsibilities

  •  Under the supervision of the CQA Manager, responsible for the accuracy, and maintains integrity and confidentiality of, donor/potential donor records as per policies and regulations. Abides by data review and documentation policies and procedures, including escalation timelines for data access, receipt, correction, and handoff. 
  • Daily and designated weekend/holiday responsibilities of Donor Record QA in accordance with policies and regulations, including: 
  • Review of donor records and recordings in accordance with policies and regulations. 
  • Timely and accurate access and receipt of donor records from external entities and data submission to regulatory agencies. 
  • Assigns and escalates tasks for record correction to appropriate departments and personnel. Obtains, shares, and responds to positive culture/serology results in accordance with policies and federal/state/local laws and regulations, including any necessary reporting to LiveOnNY’s designated Patient Safety Contact. 
  • Deceased Donor Registration and Donor Disposition Feedback review and submission to UNOS. 
  • As a designated Patient Safety Contact for LiveOnNY, receives, shares, and responds to positive culture/serology results and UNOS Potential Disease Transmission Reporting notifications in accordance with policies and federal/state/local laws and regulations. Reports PDTRs and Patient Safety Events to UNOS and other regulatory agencies as indicated. 
  • Participates in necessary training for performance of assigned roles and as part of ongoing professional development; ensures accurate and timely completion of deliverables as an individual contributor. Reviews and acknowledges all new/revised policies, work instructions, and job aids as assigned. 
  • In conjunction with CQACs, provides accurate and timely post-recovery follow-up with organizational partners such as donor hospitals, transplant hospitals, ambulance/emergency services, Medical Examiner offices, and eye/tissue banks to coordinate compliance with access, review, and information sharing. 
  • Under the supervision of the CQA Manager, identifies and escalates access barriers to Donor Record QA and data transmission. Proactively collaborates with Medical Records Coordinators to identify and obtain donor records as necessary. 
  • Provides relevant information and timelines for Quality-related Occurrences as needed. Report incidents to the CQA Manager for review/submission as potential Occurrences. Responsible for documenting donor record-related Occurrences and PDTRs into LiveOnNY’s Quality Management System as directed. Participates in Occurrence-derived Interviews and Root Cause Analyses as directed. 
  • Proactively identifies areas of risk to departmental and organizational leadership, including to the Director of CQAPI and the CQA Manager. 
  • Proactively identifies existing or anticipated internal/external barriers and challenges to Donor Record QA or UNOS documentation, submission, or follow-up. 
  • Participates in team huddles and collaborative meetings, including meetings with organizational partners such as tissue processors, eye banks, and critical vendors/suppliers.
