About the Role
This position oversees a portfolio of Digital Navigation and Outreach program management, driving CHNW’s Link to Care WA program, which support healthcare access through digital navigation. This role develops and executes work plans to support development of statewide resources including the Link to Care Washington hotline and website. The Program Manager II – Digital Navigation and Outreach leads hotline activities, including enabling call support, trainings, and promotion of the hotline. Key responsibilities for this program manager are to analyze information to identify various internal and external stakeholders to outreach to develop partnerships.
To be successful in this role, you:
- Have bachelor’s degree in a relevant field (e.g., Public Health or Health Administration) or equivalent combination of education and highly relevant experience.
- Have a minimum of three (3) years of project or program management experience. Have experience managing complex data and reconciling data output/reporting.
- Have experience in leading multiple complex projects related to strategic initiatives, performance management, and business development.
Essential functions and Roles and Responsibilities:
- Responsible for managing complex cross-functional projects to advance program development, including leading the hotline workplan to achieve programmatic goals and support strategy related for hotline support, training development, and outreach.
- Collaborates with the department leadership to ensure projects goals are met, including the coordination of analytics reports, tracking progress, and adherence to timeline.
- Provides regular communication with stakeholders, ensuring project updates and the monitoring of progress against established goals.
- Conducts comprehensive research and analysis, including assessing environmental conditions, community, regional, and population needs and best practices to identify opportunities for digital navigation support with a focus on equity.
- Recommends solutions for the development of innovative or integrated programs and strategies to enhance healthcare access.
- Actively participates in cross-sector stakeholder opportunities in to provide hotline promotion and expertise on digital navigation strategy.
- Organizes cross-team and departmental collaboration with internal and external subject matter experts to develop system knowledge and support key strategic opportunities.
- Provides ongoing monitoring, process improvement, and oversight for implemented programs related to the digital navigation hotline.
- Develops and organizes program documents to further knowledge management of digital touchpoints in healthcare, ensuring continuous improvement of the digital navigation process.
- Leads outreach efforts to prospective partners to promote the digital navigation hotline, develops referral pathway to the hotline, and creates partnership materials to support these processes.
- Facilitates stakeholder engagement in external systems and strategy forums, coordinating input across relevant departments to identify key priorities, develop communications, track key deliverables, and ensure effective follow-up.
- Assures compliance with relevant regulatory agencies (OIC, DSHS, HCA, CMS) and assists in the management of contracts or grants, including monitoring the scope, schedule, and budget of projects and reporting results.
- Answer digital navigation hotline calls on a as needed basis.

Knowledge, Skills, and Abilities:
- Knowledge of healthcare landscape in Washington state.
- Ability to review and advance project deliverables for grant, strategy and business goals.
- Ability to oversee and manage multiple projects within a program, ensuring cohesion and success, and meeting deadlines. Effective verbal and written communications skills; ability to collaborate across departments for program success, fostering team work.
