Admissions Coordinator

Hillcrest Firethorn is currently seeking an Admissions Coordinator to support individuals transitioning through post-acute and long-term care settings by coordinating their journey from admission to discharge and beyond.

Key Responsibilities

• Maintains excellent communication with referral sources and potential customers providing timely and accurate information and demonstrating outstanding customer service.


• Processes all assigned referrals including screening, providing answers and completing all appropriate steps according to established protocols and workflows.


• Conducts medical review for referrals based on established admission and financial criteria for service lines.


• Conducts verification of all insurance coverage and insurance authorizations accurately and timely for all customers within Hillcrest Firethorn.


• Coordinates and visits re-hospitalized customers and reports customer status to the service line.


• Demonstrates mastery of the CRM and Electronic Health Record software as well as other software required for the position in order to process referrals timely and accurately.
Qualifications

• Licensed Practical Nurse (LPN), Registered Nurse (RN), or equivalent healthcare background preferred.

• Previous experience in post-acute, skilled nursing, or senior living environment highly desirable.

• Strong interpersonal and communication skills with a passion for customer service and relationship building.

• Ability to multitask, problem-solve, and maintain professionalism in a fast-paced environment.

• Knowledge of insurance processes, discharge planning, and care coordination is a plus.
