We are looking to find a qualified and experienced MDS resource (RN or LPN) to join our team as a Regional MDS Support and to be a leader for our skilled nursing facilities. Ideal candidate should have exceptional interpersonal and leadership skills tailored towards mentoring other nurses and setting them up for success. This position will also serve as an interim MDS Coordinator when positions have been vacated, during leaves of absence and/or vacations.
Location: This is not a remote position. Based in Woods Cross. Must live along the Wasatch Front (Utah)
Travel required between various skilled nursing facilities in Utah, Idaho, Wyoming
The primary purpose of this position is to train and develop the MDS Nurses at each facility to ensure the completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS) and Care Plan in accordance with State and Federal requirements. This position requires travel between our various skilled nursing facilities.
This resource will also help facilities in Quality Measures, State Case mix, and other CMS regulatory requirement.

MDS Resource Qualifications:
Preferred: 3-4 years’ experience working in a long-term care / skilled nursing setting as an MDS Nurse
Proven track record with improving/maintaining 5-star quality measures for the facility
Proven track record with improving/maintaining case mix for the facility
Utah or Compact License RN or LPN license is required
Experience with RAI process and MDS certification preferred.
Professional, dedicated, and knowledgeable with solid communication and teaching/training skills.
Experience:
MDS 3.0 RAI: 1-2 years (Required), 3-4 years (Preferred)
PDPM: 1 year (Preferred)
License/Certification: RAC-CT preferred
LPN or RN License (Required)

Qualifications
Required Qualifications
- A current, unencumbered, active RN or LPN license is preferred.
- Experience with MDS completion preferred.
- Resident Assessment Coordinator (RAC) training must be completed within six months of hire. RAC Certification is preferred.
