Job Description:

As a Hospice LCSW, you'll play a pivotal role in our interdisciplinary team, providing comprehensive psychosocial assessments, counseling, and support services to patients and families coping with terminal illness. You'll collaborate closely with physicians, nurses, chaplains, and other members of the team to develop individualized care plans that address the emotional, social, and spiritual needs of our patients.

Key Responsibilities:

· Conduct psychosocial assessments to identify patient and family needs, strengths, and resources

· Provide counseling and emotional support to patients and families facing end-of-life challenges

· Assist with advance care planning, including discussions around goals of care, end-of-life wishes, and decision-making

· Facilitate family meetings and support groups to promote communication and coping strategies

· Advocate for patients' rights and ensure their preferences are respected throughout the hospice journey

· Collaborate with community resources to address financial, legal, housing, and other social determinants of health

· Participate in interdisciplinary team meetings to coordinate care and optimize patient outcomes
