Clinical Social Worker - Moms do Care, 40 hour Days

This position is part of the High Risk Intervention Team (HRIT) providing psychiatric services and connection in the community for our clients enrolled with the Compass Wellness Program.

As a member of the Maternal and Newborn High Risk– Compass Moms D Care Program, the LCSW/LICSW will work directly with patients, families, caregivers and providers. The LCSW/LICSW will assess patients to identify signs of behavioral health disorders, including substance use disorder and will help individuals deal with addictions and substance abuse; family and parenting problems; stress management; and maternal or post-partum mental health concerns.

The LCSW/ LICSW will also provide support to the Maternal and Newborn Health - Moms Do Care program. The LCSW/LICSW will work collaboratively with outpatient therapy providers to ensure patients have longer term support in the community. The LCSW/LICSW will report to the Program Director of the Compass Moms Do Care Program.
