This position is part of the High Risk Intervention Team (HRIT) providing psychiatric services and connection in the community for our clients enrolled with the Compass Wellness Program.As a member of the Maternal and Newborn High Risk– Compass Moms D Care Program, the LCSW/LICSW will work directly with patients, families, caregivers and providers. The LCSW/LICSW will assess patients to identify signs of behavioral health disorders, including substance use disorder and will help individuals deal with addictions and substance abuse; family and parenting problems; stress management; and maternal or post-partum mental health concerns.
The LCSW/ LICSW will also provide support to the Maternal and Newborn Health - Moms Do Care program. The LCSW/LICSW will work collaboratively with outpatient therapy providers to ensure patients have longer term support in the community. The LCSW/LICSW will report to the Program Director of the Compass Moms Do Care Program.
