As a Registered Nurse on the FACT team, you will collaborate closely with our multidisciplinary team to provide a range of treatment, rehabilitation, and support services to our clients. Your role will involve assessing physical health needs, administering medications under the direction of the psychiatrist, coordinating medical assessments, and contributing to the overall well-being of our clients. Your expertise will play a vital role in enhancing clients' quality of life and supporting their journey to independence.

As a Registered Nurse on the FACT team, you will collaborate closely with our multidisciplinary team to provide a range of treatment, rehabilitation, and support services to our clients. Your role will involve assessing physical health needs, administering medications under the direction of the psychiatrist, coordinating medical assessments, and contributing to the overall well-being of our clients. Your expertise will play a vital role in enhancing clients' quality of life and supporting their journey to independence.
Primary Responsibilities:
- Develop, maintain, and supervise team psychopharmacologic and medical treatment policies and procedures in collaboration with the team medical provider.
- Administer and evaluate prescribed psychotropic medications, monitor their effectiveness, and manage potential side effects.
- Coordinate and administer medical assessments, making referrals to community physicians and dentists as necessary.
- Provide substance use education to clients, utilizing a harm reduction approach.
- Offer psychiatric rehabilitation services to clients and assist in developing essential life skills.
- Maintain accurate and timely documentation of client progress and relevant nursing services.
- Participate in daily staff meetings and treatment planning reviews.
- Collaborate with the team in crisis intervention and risk reduction efforts.
