Community Health Worker Grant - Community Outreach - St. Vincent Medical Center

Pathways Program: Provide care coordination for program-eligible pregnant women to help ensure they have a medical home for prenatal and post-partum care, practice appropriate pregnancy self-care, get connected to needed social services to remove barriers to care, and learn basic maternal and baby care with the goal of achieving a full term, healthy birth weight birth. Care coordinators educate clients on topics including safe sleep practices, benefits of breastfeeding, and dangers of tobacco use. Services are clinic and home-based.

Preferred: Certified Community Health worker (if not certified, be willing to obtain within first year of hire). Required: High School diploma or GED, valid Ohio Driver's License and reliable transportation. Should have good oral and written communication skills, enjoy working with people, be a self-starter, have knowledge of community resources and have good computer skills.
