About the role

Job Summary
Under the supervision of the Director of Integrated Harm Reduction and Linkage to Care Services, the registered nurse has the responsibility of providing care coordination and direct health care to patients seeking HIV prevention, including HIV Postexposure prophylaxis (PEP) and HIV Preexposure prophylaxis (PrEP), HIV testing, Hepatitis C testing, and STI screening. The RN will also provide direct care from a harm reduction lens for clients including hepatitis prevention and treatment, naloxone education and distribution, safer use for people using drugs, wound care, HIV prevention and linkage to care.

This position will also facilitate client-led linkage internally within the Centers and collaborate with the interdisciplinary team including Primary Care, Pharmacy, Substance Use Disorder (SUD), Medicated-Assisted Treatment (MAT)/Medication for Opioid Use Disorder (MOUD), and Psychiatry/Behavioral Health services.

Essential Job Duties & Responsibilities
▪ Provides direct care to patients, including triage, care coordination and assessments in accordance with Ohio’s Nurse Practice Act.
▪ Identifies or targets high-risk patients within the practice that could benefit from care management support. Performs outreach to the identified clients to offer care management support.
▪ Utilizes the nursing process in the provision of nursing care including but not limited to administering treatments and medications in compliance with the practitioner(s) orders, standards of care, and applicable policy.
▪ Provides education to patients, families, and other community members regarding HIV Prevention, Narcan, and harm reduction services.
▪ Provides and assists patients and their families in problem solving potential issues related to health care, financial, or social barriers (e.g., request for interpreters as appropriate, transportation services or prescription assistance). Uses a step-by-step process when teaching skills. Uses positive reinforcement and encouragement. Uses a flexible approach with a diverse population.
▪ Provides mobile HIV prevention, harm reduction, and Narcan services to patients and their families as needed.
▪ Sets standards/templates for documentation for all care coordination and direct healthcare services. Completes required interagency and other necessary paperwork. Monitors confidentiality in accordance with health center policy and legal requirements. Follows required office procedures in a cooperative manner. Initiates and processes incident reports as per organizational guidelines. Adheres to Infection Control protocols.
▪ Participates in the Quality Assurance and Performance Improvement (QAI) process.
▪ Promotes effective coordination, communication, and teamwork between departments and among medical staff. Communicates with appropriate health care providers and social service agencies in accordance with policy.
▪ Attends departmental and The Centers’ All-Staff meetings.
▪ Contributes to departmental and organizational decision making through active participation in the process.
▪ Works effectively and positively with co-workers and volunteers.

Job Qualifications
▪ Bachelor’s degree or Associate’s degree.
▪ An unencumbered license as an RN in the state of Ohio
▪ Prefer a board licensed and certified RN from an accredited school of professional nursing.
▪ One or more years of clinical work experience in HIV services or AOD services.
▪ Ability to lift up to 25 pounds
