<p>Hi, we're Oscar Medical Group. We’re hiring an Licensed Vocational Nurse or a Licensed Practical Nurse to join our Primary Care team.</p> <p>At Oscar Medical Group, we are refactoring healthcare. Note, this list of states is subject to change. #LI-Remote</p> <p><strong>Pay Transparency:</strong></p> <p>The pay for this role is: $30 per hour. You are also eligible for employee benefits, and monthly vacation accrual up to 10 days per year.</p> <p><strong>Responsibilities:</strong></p> <ul> <li>Manage patients’ healthcare needs through traditional phone calls, video calls, and asynchronous messaging</li> <li>Maintain quality standards on all documentation and communications using the appropriate platforms.</li> <li>Communicate with clinicians and care team members frequently regarding patient care and escalate cases</li> <li>Perform transition of care outreaches to facilitate post-discharge goals </li> <li>Provide clinical assistance to determine appropriate support based on patient’s health needs</li> <li>Identify and engage barriers to achieving optimal member health and apply strategies to reduce patient risk</li> <li>Assist clinicians with administrative paperwork, fax orders or prescriptions to hospitals, pharmacies, or other vendors and schedule patient appointments</li> <li>Suggest services or preventive screenings relevant to each patient and coach patients  to reach their care goals by following medical treatment plans</li> <li>Manage pre-visit offerings by patient type and disease status </li> <li>Support development of expanded nursing services.</li> <li>Follow all member privacy and confidentiality standards of practice (i.e. HIPAA)</li> <li>Compliance with all applicable laws and regulations</li> <li>Other duties as assigned</li> </ul> <p><strong>Requirements:</strong></p> <ul> <li>Graduate of an accredited School of Practical/Vocational Nursing (LPN, LVN)</li> <li>Active, unrestricted LVN/LPN license in one of the following states: Georgia, Florida, Texas or compact/multistate license</li> <li>Experience in virtual based practice </li> <li>Spanish language fluency</li> <li>3+ years of in person outpatient or ambulatory LVN/LPN experience</li> </ul> <p><strong>Bonus points:</strong></p> <ul> <li>Experience in basic behavioral health interventions used in primary care </li> <li>Experience in healthcare, medical management, change management, and/or leadership</li> <li>Working knowledge of Milliman Care Guidelines</li> <li>Knowledge or familiarity with the principles of project and program management</li> <li>Willingness to obtain additional state licenses as needed</li> </ul><p><span>This is an authentic Oscar Health job opportunity.
