Job Summary:
Coordinates and supports activities that assess patient needs, experiences, and expectations for the purpose of satisfying and whenever possible exceeding expectation levels by implementing various tools (patient complaints and responses, surveys, focus group, home visits, etc.). Applies substantial knowledge and experience to perform a wide range of advanced activities and/or determines how to use resources to meet schedules and goals; serves as working supervisor for team or work group. Coordinates small program(s) with limited budget/impact.
Job Requirements:
Education and Work Experience:
- High School Education/GED or equivalent: Preferred
- Associate’s/Technical Degree or equivalent combination of education/related experience: Preferred
- Five years' relevant experience: Preferred
Essential Functions:
- Implements various assessment tools (surveys, focus group, home visits) to identify patient needs, experience and expectations and reviews patient responses.
- Collects, documents, distributes, organizes and translates information necessary to summarize the findings from the assessment activities of patient experience in a manner that facilitates the recognition of both the patient’s family and the patient’s contextual environment. Generates reports and summaries to assist in the interpretation of information. Assists in the preparation and submission of the industry and agency required reporting.
- Collaborates and communicates with department managers, front line staff, faculty and residents to identify process flow areas affecting the patient experience.
- Adheres to all company policies, procedures and industry standards. and agency regulations and rules.
- Performs other job-related duties as assigned.
