Schedule: Full-Time, Monday Through Friday shift with rotating on-call coverage for evenings and weekends.
The Primary Care Physician provides primary medical care services to older adult (> 55 years of age) participants of Esperanza’s Program of All Inclusive Care for the Elderly (PACE) using primary care concepts, emphasizing prevention of illness and the promotion of healthy lifestyles. Provides leadership and training to other health care professionals. Functions as a member of the PACE Interdisciplinary Team (IDT) in care coordination of PACE participants; participates in the program planning of health activities within the health center and in the community, as requested
Primary Duties and Responsibilities:
- Manages primary care medical practice in PACE Center, in participants’ homes, and other post-acute facilities, as needed..
- Performs all functions and duties as required as a member of the PACE Interdisciplinary Team (IDT), including, but not limited to, participation in IDT meetings, family conferences, grievances, service determination requests, CMS audits.
- Coordinates with IDT to assist in developing individualized plans of care for each PACE participant.
- Participates in quality improvement activities of the PACE center including peer review, and other designated projects.
- Practices sound utilization management of participant care within the PACE model.
- Obtains privileges in partner acute and post-acute facilities, participates in decisions regarding admissions, and rounds on participants in the care of these facilities.
- Maintains timely communication with partner hospitals, specialists, nursing homes, and other contracted providers regarding participants’ services, to ensure continuity of care and maintain oversight of care offerings provided.
Requirements
- Bilingual (preferred).
- Board certification in primary care required. Board certification or specialization in geriatrics preferred. At least 1 year working with older adults strongly preferred.
- Licensed as a physician in the State of Illinois.
- Must possess valid driver’s license and documentation of current automobile insurance.
- Must comply with requirements of other facilities (hospitals, LTC (Long Term Care) facilities, etc.) related to flu or other vaccines if job duties require providing services in those facilities.
