Regional Director of Care Management

The Regional Director of Care Management provides leadership and acts as a role model for all Health Home personnel in the planning and delivery of client services. The Regional Director oversees program operations and collaborates with all departments to develop, deliver and evaluate the program services provided to the Health Home clients. The Regional Director directly supervises all administrative and care management staff and collaboratively supervises all facilities and plant operations at the program site.
Responsibilities:
- Represent agency with regional LEAD HH accounts
- Responsible for the quality assurance, quality improvement, and compliance of the care management program, ensuring that regional, state, programmatic, and agency standards are met and maintained
- Direct, coordinate, and implement training programs for case management staff, including both internal training and outsourced instruction
- Build, develop, and maintain community relationships and referral arrangements with community agencies and hospitals
- Participate in intra- and inter-agency community meetings and committees as assigned
- Analyze program performance and forecasts future demand and resource requirements for continued effective program operation.
- Assure compliance with all policies, regulations and laws governing clients’ rights and confidentiality of information
- Monitors daily performance, weekly reports, and monthly reports
- Helps to recruit, hire and train staff for regional team
- Helps formulate marketing plans to support growth of all programs and services across all lines of business
- Supervises Account Management team and other marketing staff to ensure targets are achieved
- Helps develop account specific plans and strategies and guides account managers in implementing plans
- Participates in Agency workgroups to address business development as needed
- Meets regularly with Operations management to ensure business development is working collaboratively to support achievement of operational goals
- Performs all duties inherent in a senior management role including but not limited to participating in recruitment, hiring, supervision, performance evaluations, training, disciplinary actions and termination

Requirements:
- Bachelors Degree Required
- Masters Degree Preferred
- Director Care Management experience required.
Job Type: Full Time
Pay: $71,847.15 to $89,808.93
