Your Specific Responsibilities:

The incumbent an employee of the Florida Department of Health in Clay County works to protect, promote and improve the health of all people in Florida through integrated state, county, and community efforts. Program Oversight:

  • Communicates with Clay County District Schools (CCDS) to ensure adequate program oversight as it relates to immunization compliance and record reviews.
  • Provides annual updates on School Health Services Manual and other reports, as assigned, verifying information for accuracy, completeness, correct spelling, punctuation, and grammar usage.
  • Conducts virtual and in-person immunization and school health entry record reviews for all the schools in CCDS, as assigned.
  • Conducts immunization compliance reporting during pre-FTE week, FTE week, monthly and as needed, in CCDS.
  • Updates and creates internal forms for the School Health Services Program, as assigned by supervisor.
  • Provides consultative and technical guidance to the CCDS, for the development of their forms.
  • Assists with collecting, analyzing, and inputting of data/statistics for School Health Services Annual Report, School Health Services Plan, school district monthly reports, screening data, annual kindergarten and seventh grade immunization reports, end of the year reporting or any other report, as assigned.
  • Updates information in the sub-folders that are part of the School Health Services Manual and/or are used by the School Health Services Program. 
  • Conducts school visits, attends community meetings or community outreaches utilizing a county vehicle or own vehicle, as per policy.
  • Performs other related duties as required/assigned and within the realm of licensure, protocols, and the classification specifications of a Senior Licensed Practical Nurse.

 

Collaboration:

  • Effectively communicates with CCDS school health designees (RN, LPN, Aide), principals, social workers, registrars, guidance counselors, and any other school staff.
  • Collaborates with the schools to collect student specific health care information to assist DOH-RN in coordinating care between school staff, parents, and healthcare providers to assure a healthy learning environment for the students.
  • Collaborates, organizes, and participates in outreach events with community partners, as assigned.
  • Assists DOH Clay Epidemiology Program with outbreak investigations, upon request.
  • Attends and participates in DOH meetings, school health monthly staff meetings and any other meeting and committees, as assigned. May be asked to take notes during any mentioned meetings and provide updates to the school health team that come from community committees.
  • Active member of the School Health Wellness Advisory Committee (SHWAC) and community committees, as assigned.
  • Communicates with parents and other professionals by telephone, or letter for follow-up of health problems or continuity of care.

 

Health Screenings:

  • Supports CCDS in completing health screenings, which can include scoliosis, vision, hearing, and BMI, as available.
  • Conducts health screening training for school nurses, DOH staff and school volunteers.
  • Ensures that CCDS health designee completes rescreens and screening of absentees, per state designated time frame.

 

Trainings:

  • Trains registrars, and CCDS health designees on current immunization and school entry health exam requirements.
