The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Nurse Practitioner (NP)/Physician Assistant (PA), is responsible for providing primary health care to individuals and families who will be accessing care at Stout Street Health Recuperative Care Center (SSRCC), or any of our other clinical sites. This position is for a PRN NP/PA, mostly working weekends and holidays. The minimal requirement is 2–10-hour shifts per month. The NP/PA would provide high-quality acute, chronic, and preventive care across the full age span, and works collaboratively with the SSHC multidisciplinary integrated health team.

Essential Job Functions
- Provides direct, high-quality, primary medical care to people experiencing homelessness.
- Maintains and enhances a current level of knowledge relevant to professional practice and continually strives to improve the quality of care.
- Breathes the spirit of Motivational Interviewing, Harm Reduction, and Trauma-Informed Care with clients, and provides information and choices in a non-judgmental, respectful manner.
- Maintains a work environment that is marked by respect for others, that values inclusiveness and builds workforce diversity, and that fosters cooperation and teamwork.
- Collaborates and consults effectively and bi-directionally with all CCH staff and identifies and addresses the complex issues which may adversely impact the health of patients.
- Maximizes community linkages while acting as advocate for patients.
- Participates in Quality Improvement and Peer Review activities.
- Maintains Continuing Medical Education, license (s), and CCH Credentialing and Privileging requirements always.
- Delivers care across CCH sites which may include outpatient clinic sites, the mobile clinic, Stout Street Recuperative Care, street outreach or home visits. Weekend /evening/Holiday coverage is required. Performs other duties as assigned.
Qualifications Summary
- 2 or more years of experience required for this job position. Requirements for Nurse Practitioners:
- A Master of Science or Doctorate degree in Nursing from a program accredited by the American Nurse Association or the American Association of Nurse Practitioners.
- Current registration (or ability to obtain) in the State of Colorado as an Advanced Practice Nurse.
- Certification as a Nurse Practitioner from the American Nurse's Credentialing Center. Requirements for Physician Assistant
