How You’ll Lead & Serve

• In-Home Clinical Oversight
  
   
  • Complete start-of-care assessments and craft personalized, compliance-ready Care Plans.

  • Revisit clients on-site to update goals and interventions as conditions change.

• Caregiver Coaching & Compliance
  
   
  • Orient new aides to each Care Plan; perform skills check-offs and annual competencies.
    
    
    
• 
  
  • Audit field documentation, spot gaps, and provide real-time feedback that elevates care quality.
    
    
    
• 
  
• Recruitment & Retention Partner
  
  
  • Collaborate with HR to interview, select, and mentor top-tier caregivers; champion a culture of recognition and growth.
    
    
    
• Documentation & Reporting
  
  
  • Maintain HIPAA-secure records, track condition changes, and escalate concerns to physicians and leadership.
    
    
    
• Family & Community Liaison
  
  
  • Build trusting relationships with clients, their support networks, and referral sources; represent our agency’s mission wherever you go.
    
    
    

What You’ll Bring

• Active Registered Nurse (RN) license in CA.
  
  
  
• Bachelor’s in Nursing or related field – OR – 2 + years’ geriatric / home-care case-management experience.
  
  
  
• Proven ability to juggle multiple home visits, phone triage, and documentation with precise time management.
  
  
  
• CPR/First-Aid certification and valid driver’s license.
  
  
  
• Warm, decisive communicator who inspires confidence in caregivers, clients, and families alike.
