The Department of Medicine, Division of Infectious Diseases has an exciting opportunity for a Sr Case Worker. The Sr Case Worker  serves as the primary development resource for patients and delivers direct social services to patients. The incumbent works in close collaboration with medical doctors and other members of the health team to further their understanding of significant social, emotional, and developmental factors affecting patients.

• Responsible for arranging supportive services for clients including assisting with Ryan White/Insurance application, medical related services, identifying available resources, setting up transportation assistance and referrals.  
• Follow- up with all referrals given by the case worker and medical providers to ensure that access to services has occurred.
• Conduct initial assessments to identify needs and develop a care plan.
• Coordination of all services required to implement the plan.
• Client monitoring to assess the efficacy of the plan on a minimum of a monthly basis.
• Conduct assessments and re-assessments as mandated by funding contracts. Adhere to funding contract requirements.
• Work collaboratively with medical, mental health, substance abuse and community service providers as well as any other member of the client treatment team.
• Input accurate and complete documentation of activities through case notes, assessments, and other forms required by the funding contract.
• Advocate for appropriate services for clients based on needs and assessments.
• Foster client independence through education, supportive services, and other methods of empowerment and efficacy building.
• Conduct visits with clients in various settings, including in the community, client’s home and attending client appointments.
• Develop relationships in the community to gain access to People Living with HIV (PLWH) and high-risk negatives.
• Ability to work effectively with humans of diverse gender identities, races, ethnicities, ages, sexual orientations, and drug use patterns, in a multicultural environment.
• This is a critical component to the agency and the work that is done in the community. Willingness to work non-traditional hours.

Minimum Requirements:

Bachelor’s degree in relevant field

Minimum 3 years of relevant experience

Bilingual preferred (English/ Spanish or English/Haitian creole).

• Ability to analyze, organize and prioritize work under pressure while meeting deadlines.
• Ability to process and handle confidential information with discretion.
• Ability to work evenings, nights, and weekends as necessary.
