Job Summary: Hospice clinical leader providing oversight of clinical department(s), team(s) and/or program(s) including budgeting, staffing, quality of care, education and training, and program development. Processes Electronic Medical Record (EMR) workflow. Processes payroll. Responsible for patient satisfaction, quality of care, productivity, and staff satisfaction. Responsible for program development to enhance existing or new programs.

Job Duties:

  • Instructs staff on their responsibility in following the Compliance and Ethics Program.
  • Detects noncompliance and follow up as appropriate.
  • Responsible for the overall hospice clinical operations of assigned territory, coordinating high quality care.
  • Develops and coordinates hospice clinical orientation and training to staff members in collaboration with education and managers. 
  • Familiar with EMR documentation and provides leadership and mentorship to managers.
  • Monitors and provides support throughout orientation period, and longer if determined to be necessary.
  • Provides training on clinical practices, regulations, policies and procedures for nursing staff in coordination with the Executives, Human Resources, Infection Control, and Education.
  • Ensures completion of initial and yearly clinical competencies for clinical employees.
  • Develops and implements additional formal classroom and field training as needed to ensure compliance with changes to practice. 
  • Educates staff on the development of plans of care, including individualized interventions and measurable outcomes for goals. 
  • Creates and updates clinical training materials as needed.
  • Serves as clinical resource for problem solving, time management, and skill development for managers and team staff. Informs Chief Operating Officer of any performance, training, or quality of care issues within the team.
  • Maintains flexible work hours to meet the education and staff needs.
  • Routinely audits clinical documentation for completeness and appropriateness of documentation, indications of POC driven service, timeliness of documentation, interdisciplinary communication, continuity of care, and compliance with policies, best practices, and regulations in collaboration with managers.
  • Focuses on patient satisfaction and develops key metrics for satisfaction.
  • Responsible for quality improvement and develops PIPS to ensure quality measures are in place. Conducts Root Cause Analysis on identified issues.
  • Focus on end of life measures through assessment and MUSE tool. Maintain HVLDL quality scores higher than national averages. 
  • Responsible for CAHPS scores meeting board goals. Develops programs to enhance services for patients.
  • Overall accountability for pharmacy and supply expenses for areas of supervision.
  • Responsible for oversight of regulatory compliance including Joint Commission for hospice, federal, state, and other applicable regulatory agencies within their scope of care.
  • Develops and enforces staffing plans for all areas of supervision.
  • Overall accountability for departmental goals and directing staff and teams under areas of supervision.
  • Other duties as assigned

Qualifications

Education and / or Experience: Bachelor’s degree from four-year college or university required; Master’s degree preferred: at least 5 years hospice experience required. Graduated from an accredited school of nursing. At least 5 years of management experience required.
