The Emergency Department at Hartford Hospital serves as a critical entry point to care as a Level 1 Trauma Center and a regional destination quaternary care hospital. Hartford Hospital is seeking a Full-Time Evening Emergency Department Health Social Worker to join our interprofessional team. In this role, the social worker provides holistic, patient-centered, and family-focused care, partnering closely with physicians, nurses, and other team members in the fast-paced emergency setting.
The Emergency Department Health Social Worker addresses the psychosocial impact of acute and serious illness, trauma, and injury while helping patients and families navigate complex medical and social systems. The role is central to identifying and responding to Social Determinants of Health, assessing mental health needs, supporting coping and safety, to promote continuity of care and wellbeing.
This position is ideal for a clinician who thrives in a collaborative environment, values trauma-informed and equitable care, and is committed to supporting patients and families at moments of significant stress and vulnerability.
Job Summary:
Provide complex social work services as a member of an interprofessional team; care that is informed by comprehensive biopsychosocial-spiritual assessment, intervention and treatment. Demonstrates strong advocacy skills; Requires activated and engaged social worker exerting discretionary effort. Values holistic patient-centered and family-focused care. Will work effectively on a complex interprofessional team.
