Broad Street Home Care is seeking Registered Nurses and LPN's to work with private clients in a one-on-one home setting located in Chicago. About The Role We are seeking a dedicated and compassionate In-Home Nurse with specialized experience in caring for patients with neurodegenerative diseases and managing enteral feeding (G-tubes, J-tubes, etc.). This role provides one-on-one nursing care in a patient’s home, helping maintain their dignity, comfort, and quality of life. This position works closely with the Nurse Care Manager to ensure the client receives the proper care and support needed to maintain independence and remain safely in their home. Location: Chicago Schedule: 12-hour Day Shift Position Requirements

Current Illinois Nursing License
4 years or more of Nursing Experience
Must possess the ability to navigate a phone application to clock in and out, as well as enter case notes
Demonstrate the ability to be a strong team player
Ability to work independently, and when circumstances warrant, the ability to make decisions
Previous home health experience preferred but not required
Provide skilled nursing care to patients in a home environment
Manage and monitor feeding tubes and nutritional plans
Administer medications and treatments per care plan
Assist with daily living activities, mobility, and hygiene
Monitor for changes in condition and report to the care team
