Position Title:                 

• WVGTG Transition Coach

 
Job Description:

• Collect and analyze data on SWDs ages 14-21
• Refer students at risk of failing to the appropriate services (tutoring, mental health, IEP Team, Attendance, etc.)
• The Transition Coach will work directly with CIS Coordinators, Guidance Counselors, Social Workers at the school locations
• Follow up to determine success or need for additional services.
• Discuss careers and soft skills with students
• Review transition plans for IEPs, utilizing the Transition Plan Checklist to ensure compliance.
• Assist Transition Assessments
• Assist with completing the Summary of Performance for graduation students.
• Assist with one-Year Follow up Surveys

 
Qualifications:              

• Professional Teaching Certificate to cover grades 5 -12.
• At least five years teaching experience in grades 5 -12.

 
Effective Date:             

• 2026-2027 School Year

 
Salary:                       

• Hourly rate of $25 for 90 hours per month, not to exceed allotted grant amount.
