Job Purpose or Objective(s): As the Case Manager, you will work directly with the Probation Office and Behavioral Health, supervising clients under the District Court’s supervision, ensuring compliance with mental health treatment plans, providing support to facilitate rehabilitation and reintegration into society. You will report to the Behavioral Health Care Manager.

Primary Tasks:
1. Work directly with the Probation Department and Behavioral Health for dual supervision of clients who have been diagnosed with certain mental health diagnoses and/or substance abuse, who need assistance adhering to a prescribed medication or rehabilitation regimen.
2. Develop and implement individualized case management plans.
3. Monitor clients’ progress and compliance with court-ordered conditions by making field visits.
4. Coordinate with healthcare providers to verify medication adherence.
5. Coordinate with any rehabilitation facilities or sobriety programs to verify attendance.
6. Provide support and resources to clients to help them access mental health services.
7. �� Facilitate connections to tribal and community resources and support networks.
8. Maintain accurate and up-to-date records of clients’ progress.
9. Prepare reports for court hearings and probation reviews.
10. Document all interactions and interventions with clients.
Required Education and Experience:
· Bachelor’s degree or higher in Psychology, Social Work, Counseling or related human services
· Intermediate general technology and MS Word processing software skills
· Understand the process of crisis stabilization and crisis response
· One [1] year of experience in a behavioral health field providing rehabilitation services to children, adolescents, and adults
