Are you an experienced and compassionate Registered Nurse (RN) looking for a rewarding opportunity? Elara Caring is seeking an enthusiastic, dedicated RN to join our Adult Family Living (AFL) team in Connecticut! In this exciting role, you'll have the chance to make a real difference in people's lives by providing top-notch care in a family home setting. You'll collaborate with families, caregivers, and healthcare professionals to create a warm, supportive environment where individuals can thrive!

What You'll Be Doing:

• Provide Exceptional Care: Deliver high-quality nursing care, including assessments, medication management, wound care, and support for chronic conditions right in the home!
• Create Personalized Care Plans: Develop, implement, and review individualized care plans that ensure each resident gets the care they deserve.
• Keep Residents Healthy: Monitor health, administer treatments, and track progress to ensure everyone is feeling their best!
• Support Families: Empower family caregivers by providing education and training on the best practices for mobility, nutrition, hygiene, and more!
• Advocate for Residents: Be their voice, ensuring their rights, dignity, and preferences are always respected!
• Collaborate with a Rockstar Team: Work hand-in-hand with case managers, social workers, and physicians to coordinate care and adjust as needed!
• Respond to Emergencies: Be ready to step in during medical emergencies to ensure the safety and well-being of our residents.
• Make a Lasting Impact: Participate in ongoing quality improvement initiatives to help us maintain the highest standards of care!

What We’re Looking For:

• Valid Registered Nurse (RN) license in Connecticut.
• At least 1 year of nursing experience (experience in home health, geriatrics, or working with developmental disabilities is a plus!).
• A deep understanding of medical conditions, medications, and nursing procedures.
• A true passion for helping others and improving lives in a home care setting.
• Excellent communication skills with the ability to train and support family members and caregivers.
• Flexibility and adaptability to meet the unique needs of each individual.
• Current BLS (Basic Life Support) certification is required, ACLS certification is a plus!
• EMR (Electronic Medical Records) experience is a bonus.
